Prompt completion of your eAdmission will ensure your admission is not delayed. Your surgery can not be confirmed until this document is completed.
The following infiltration is required to complete your admission:
- Hospital: Darwin Private Hospital
- Doctor: Cameron Scott
- Procedure or reason for this admission: This information is contained in the Surgical Consent Form (purple form) given to you by Dr Scott.
- Proposed admission date: Date confirmed by Territory Medical Group staff
- Day Stay or overnight stay: As advised by Dr Scott
The purple and white Healthscope Consent for Medical Treatment was given to you during your consultation. This MUST be with you on the day of surgery. Failure to have this form with you may result in a delay at admissions.
Important: Please follow any instructions regarding the medications Dr. Scott prescribes to you. Proper adherence is essential for your health and the overall effectiveness of your treatment.

- Arrange your own transport to and from the hospital
