Prompt completion of your eAdmission will ensure your admission is not delayed. Your surgery can not be confirmed until this document is completed.
The following information is required to complete your admission:

- Procedure or reason for this admission: This information is contained in the Fee Estimate provided to you by Dr Wang.
- Proposed admission date: Date confirmed by Territory Medical Group staff
Important: Please follow any instructions regarding the medications Dr Wang prescribes to you. Proper adherence is essential for your health and the overall effectiveness of your treatment.

2. Anaesthesia Fees – Dr Zaki Ibrahim
Dr Zaki will be administering your anaesthesia on the day of your surgery. He will be in contact with you a few days prior to your scheduled theatre and advise you of the time to attend Darwin Day Surgery.
For all wisdom teeth removal the gap payment is approx. $400. If you do not have health fund cover the full payment is $900
3. Post Operative
A post operative appointment with Dr Wang would have also been advised by our staff. If you do not have a post-op appointment arranged within 1 week from your surgery please call 89484333 to arrange now.
This appointment will be at Territory Medical Nightcliff – Nightcliff Shopping Complex, 159 Dick Ward Drive, Nightcliff
4. Fees
Fees associated with your surgery are payable to three (3) different entities (Dr Wang’s, Dr Ibrahim and DDS). Each provider are wholly responsible for their billings.
A complete tax invoice/receipt will be issued post-surgery for any Medicare and Private Health reimbursement
You will need to talk to your private health fund regarding rebates as Dr Wang does not have the ability to process the claim on your behalf
5. Cancellations
You must advise us immediately if there are any changes to the appointments you have made including your procedure date/s. Non-attendance without prior notification may result in fees.
ADF Members: You must advise your defence health centre of any cancellations also.
