Introduction
The terms and conditions are to provide information to you, and your candidates, on how your personal information (which includes your health information) is collected and used within our practice, and the circumstances in which we may share it with third parties. How the financial consent is obtained and terms and conditions of payment for your financial consent.
Why, when and to whom is financial consent given?
Practitioners have appointed Territory Medical Group (TMG) to invoice and collect all services fees payable on behalf of the practitioner’s services.
TMG will invoice and collect all Services Fees payable by the Client unless otherwise stated. These Services Fees are exclusive of GST. Services fees may include costs for consumables including but limited to vaccines. Services Fees may include but not limited to fees accrued by pathology diagnostic laboratories and radiology
Service fees
The Client or candidate must complete a financial consent document prior to the medical assessment date. As such they are undertaking responsibility for fees incurred in respect to TMG services provided.
Invoicing
TMG will issue the Client or candidate with a valid tax invoice for the Services provided (plus applicable GST)
Payment terms
The party responsible for the financial consent must pay the service fee invoice within 30 days of the invoice date using the payment method advised by TMG
Appointments and bookings
The Client or candidate are responsible for pre-booking appointments. If a Candidate arrives without a pre booked appointment, TMG may treat that person as a private patient and may require an advance cash payment for the appointment on the day.
Cancellation and fees
If Services to be provided by TMG on a certain date i.e. Medical Assessment service date, are cancelled by the Client with less than 24 hours prior to the Medical Assessment service date, or the Candidate does not attend, TMG may charge a minimum fee of $148.00 to cover the cost of the cancelled services
Patient records
remain the property of TMG in accordance with the Privacy Act 1988.
Patient confidentiality
Each party must:
- keep any Confidential Information in respect of the other party secret and confidential, except to the extent that the party is required by law to disclose;
- take all reasonable and necessary precautions to maintain the secrecy and prevent the disclosure of any Confidential Information in respect of the other party; and
- take all reasonable and necessary precautions to maintain the secrecy and prevent the disclosure of any Confidential Information in respect of the other party; and
- not disclose Confidential Information in respect of the other party to any third party without first obtaining the written consent of the other party.
Workplace Health and Safety
If the Services (or any part of them) are performed at the Client’s premises, or on sites controlled or managed by the Client, the Client must ensure that all it complies with all applicable Workplace Health and Safety laws and standards necessary to ensure a safe work environment for TMG employees, Practitioners and other representatives.
Policy review
Terms and conditions will be reviewed regularly to ensure it is in accordance with any changes.
