Please follow these 5 important steps prior to your medical assessment
Step 1: Documentation
Please complete patient registration 48 hours prior to your medical.
Step 2. Functional Capacity Assessment Declaration
Assessment will not be conducted without completion of this declaration.Step 3. Appointment Location
In order to arrive on time for your medical please familiarise yourself with our location and parking options.Step 4. Rescheduling and Cancellation policy
If Services to be provided by TMG on a certain date (‘Service Date’) are cancelled by the Client: less than one business day before the Service Date, or the Services cannot be provided on the Service Date because the Candidate does not attend, TMG will charge a fee of minimum of $200 or up to the full cost of the cancelled services
